There are many important aspects of doing a home health assessment that will allow us to create a customized service for you or your family member. Our Nurse and Team Leader will be performing the assessment. During the assessment we will review and discuss the following topics:

Observation of client’s condition: limitations of mobility, what daily activities can they do on their own: Housekeeping, meal preparation, dressing, and personal hygiene.

Memory Loss: Taking medications on time, remembering to eat and take fluids.

Home Safety: Are there any obstacles that that could cause someone to fall, need hand rails for stairways or bathrooms.

Transportation: Access to essential services such as medical care, grocery shopping, community involvement and visiting family and friends.

After the assessment our Nurse will be leaving a folder and a spiral note book with the client. The folder will have a copy of the service agreement, Notice of Privacy Practices, Patient’s Right and Responsibilities, Home Safety Sheet and Aide Assignment Sheet covering the clients care and home duties.
